- Six sets of the representation of the design (drawings or photographs).
- Information on how the applicant has derived the right to the design from the author (normally by way of assignment, employment or other agreement).
- Certified copy of the priority document if claimed.

Industrial Design Assignment
- Power of attorney signed by the assignee.
- Deed of assignment document signed by the assignor and the assignee.

- Power of attorney simply signed.
- Merger agreement document.

Industrial Design Recordal of Change
- Power of attorney reflecting the new name or address simply signed.
- Certified copy of the certificate of incorporation or the extract from the commercial register reflecting the change.
