Filing Requirements
- Power of attorney simply signed.
- Certified copy of the extract from the commercial register or any other official documents proving that the person who signed the power is authorised to do so. If it is not possible to present such document, the power should be accompanied with the Notary Certificate confirming the authorisation of the power signor. The Notary Certificate should be additionally legalised. Moreover, all documents prepared in foreign languages must be translated into Polish language.
- Certified copy of the priority document if claimed.
Notes
- Late filing of documents: allowed within 1 month from filing date without penalty.
- Extension of time for late filing of documents: not allowed.
If the application concerns a device or a word and device mark then 5 copies of the trademark (plus 2 additional copies in black and white if the trademark is coloured) should be filed together with the application.
Registration with Customs Duty
- Power of attorney simply signed.
- Photocopy of the registration certificate.
- Certified copy of the extract from the commercial register confirming that the signor of power is authorised to act on behalf of the trademark holder with Polish translation.

It is possible to file the application for the EU customs protection.
Renewal Requirements

Recordal of Change
- Power of attorney under the new name or address.
- Certificate of incorporation or extract from the commercial register reflecting the change of name or address with Polish translation.
