- Power of attorney simply signed.
- Certified copy of the extract from the commercial register with Polish translation if the applicant is a company.
- Certified copy of the priority document with Polish translation if claimed.
- Three sets of the drawings.
- Application containing designation of the applicant(s), indication of the design’s characteristics, designation of the design’s creator(s), and indication of the basis of the design right if the applicant is not the creator.

Annuity Requirements
- Payment of the annual fees.
