- Power of attorney simply signed.
- Description, title, claims, abstract, and figures.
- Statement of ownership (this applies if the applicant is not the inventor).
- Priority document of earlier applications if applicable.
- The name and address of the applicant, the name(s) and address(es) of the inventor(salong with their nationalities. The applicant should state how the patent was assigned to him (contracts of employment or by virtue of an assignment).
