- Power of attorney legalised.
- Photocopy of the ID of the person submitting the application (author or agent).
- 3. Photocopy of an ID proof of the author if the author is a natural person.
- Two copies of the work, with a label showing the title of the work, the author's name, and the author's short bibliography.
- For written works, the work must be packaged in the form of a small booklet.
