- Power of attorney signed, sealed, and countersigned by two witnesses of the applicant/company.
- Application form.
- A document approving the ownership of such work.
- Detailed description of the creation.
- Four copies of the work.
Notes
- Late filing of documents: not allowed.
Any owner of a work, sound recording, performance, or broadcasting wishes to protect the creation, shall submit an application to the Registrar in the prescribed format with evidence. Upon receipt of the application, the Registrar shall examine it and order the applicant, if required, to submit additional documents.
