- Type of the author (sole person, a company).
- Name of the author.
- Contact details (numbers and email).
- Nationality of the author.
- Photocopies of the author's passport and ID card.

If the applicant is not the author, then we need details of the assignee as follows:
1. Type of the assignee (sole person, a company);
2. Name of the assignee;
3. Contact details (numbers and email);
4. Nationality of the assignee;
5. Copy of the commercial register, and
6. Deed of assignment document legalised.
A file containing the following:
1. Source code of the work with an explanation of the related screens and forms;
2. Name of the programing language used in the development;
3. Database schema diagram;
4. If the applicant uses web services to connect to an external database, then an explanation of the same is required;
5. Brief description of the development work-frame, and
6. Explanation of the system's screens and forms (UI) and its related user roles.
Please note that the file size should not exceed 80 Megabytes.
